Case Study 6: Acute Angle Closure GlaucomaScenario:%u2022A 65-year-old woman presents to the emergency department with sudden onset of severe right eye pain, nausea, and vomiting. She reports blurred vision and seeing halos around lights. On examination, her right eye is red, the pupil is mid-dilated and nonreactive, and the cornea appears hazy.Clincher:%u2022 The combination of acute severe eye pain, nausea, blurred vision, and a mid-dilated pupil in an elderly patient strongly suggests acute angle closure glaucoma.Explanation:%u2022Acute angle closure glaucoma is an ophthalmic emergency characterized by a sudden increase in intraocular pressure due to obstruction of aqueous humor outflow. The classic symptoms include severe eye pain, nausea, vomiting, and visual disturbances such as blurred vision and halos around lights. The mid-dilated, nonreactive pupil and hazy cornea are typical examination findings. Immediate treatment with medications to lower intraocular pressure and laser iridotomy is crucial to prevent permanent vision loss.
